Job summary

Prairie Community Services, Inc. is seeking a Direct Support Provider to supervise, care for, and keep safe individuals served in daily living activities, recreation, and community orientation. Starting wage is $17.00-$18.00 per hour.

This part-time, on-site position includes benefits and requires the ability to read/write English, a valid driver's license, and DHS background clearance.

Qualifications

• Minimum age 17+ or per licensure.
• Must pass DHS background clearance.
• Must have a valid driver's license.
• Read, write, and understand English in business context.
• Follow person served's program-related plans and procedures.
• Deliver support based on person-centered services.
• Chart daily program information when needed.

Responsibilities

• Provide supervision, care and safety to individuals served.
• Assist with daily living activities, recreation and meals.
• Support transportation, laundry and housekeeping as needed.
• Document daily program information and progress.
