Job summary

Out of the Box Solutions, LLC in Oregon City seeks a dedicated Direct Support Worker to provide one-on-one support to individuals with ID/DD. You will implement personalized service plans, help with daily living skills, and facilitate community engagement.

Requirements include a high school diploma, CPR/First Aid certification, at least 1 year of direct care experience, reliable transportation, and the ability to pass background checks.

Qualifications

• High school diploma or equivalent is required.
• 1 year of direct care experience is required.
• CPR/First Aid certification upon hire.
• Must be 18 years or older and have reliable transportation.
• Pass all state background checks and meet ODDS eligibility requirements.

Responsibilities

• Deliver training, support, and assistance in personal care, recreation, daily living skills, social skills, health needs, and rights enforcement.
• Follow and monitor the client’s individualized support plan.
• Complete documentation including clocking in/out and weekly progress notes.
• Ensure health and safety and respond to emergencies when needed.
• Coordinate with families and staff to meet client goals.
• Assist with cooking, cleaning, medication assistance, appointments, shopping, and more.
• Document progress notes each shift.
