Out of the Box Solutions in Beaverton, Oregon is looking for a Direct Support Professional to provide one-on-one support to individuals with ID/DD, helping with daily routines and community involvement. You will follow individualized care plans, document progress, and communicate with families to ensure consistent, respectful care.

Qualifications

• 18+ years old
• High school diploma or equivalent
• 1+ year of caregiving or related experience
• Valid driver’s license, insurance, and reliable transportation
• Smartphone required
• Must be able to pass all state-required background checks and meet ODDS requirements
• CPR/First Aid/AED (or willing to obtain)

Responsibilities

• Support daily routines (cooking, cleaning, personal care with the client)
• Help clients get into the community and stay engaged
• Follow care plans and goals
• Communicate with families and your team
• Complete notes and documentation
• Ensure safety and well-being at all times
