Job summary

Alpine Home Health MN is seeking dedicated care staff to provide direct care and support for individuals with disabilities. You will implement care plans, accompany clients to appointments, and maintain thorough documentation of services.

Strong communication and the ability to work independently or as part of a team are essential. A valid MN DHS background check is required, and flexibility to work weekends and holidays is expected.

Qualifications

• Excellent communication and interpersonal skills required.
• Ability to work independently and as part of a team.
• Must pass MN Department of Human Services background check and not be excluded from Federal or State healthcare programs.

Responsibilities

• Provide direct care and support to individuals with disabilities with respect and compassion.
• Implement individualized care plans and behavior support plans.
• Escort and drive individuals to appointments, activities, and errands.
• Maintain accurate, timely documentation of services provided.
• Record hours and services using the Electronic Verification Visit App.
