JD Home Healthcare in Minnesota is seeking a Direct Support Professional to provide comprehensive personal care, guided by individualized care plans, and to promote client independence through modeling and supportive supervision.

This part-time, in-person position offers flexible scheduling and a benefits package. Applicants must be 18+, hold a high school diploma or GED, pass a background check, and have reliable Minnesota- insured transportation.

Competenze

• Must be 18 years or older.
• High School Diploma or GED.
• Pass a background check with the MN DHS.
• General computer skills.
• Reliable transportation, insured in Minnesota.
• Driver's license and proof of insurance.
• Clean driving record (No DUI/DWI in past 5 years).
• Fluency in English (reading, speaking, writing).
• Dynamic communication skills.
• Working cell phone with internet access.

Mansioni

• Assist clients with behavioral and personal care services.
• Assist with activities of daily living per care plans.
• Accompany clients on recreation and leisure activities.
• Implement clients' plans as written.
• Increase clients' independence and community involvement.
• Understand and implement Behavior Support Plans and ISPs.
• Track and chart data daily per company expectations.
• Attend and engage in team meetings as needed.
