JD Home Healthcare in Minnesota is seeking a part-time Direct Support Professional to provide comprehensive personal care and support to clients in their homes and communities. You will help clients follow individualized plans, promote independence, and ensure safety.

This role requires empathy, reliable transportation, a valid Minnesota driver's license, and strong English communication. A High School Diploma or GED and ability to pass a DHS background check are required; CPR/First Aid is

Qualifications

• At least 18 years of age.
• High School Diploma or GED or equivalent.
• Able to pass MN DHS background check.
• General computer skills.
• Reliable transportation with MN license.
• Clear driving record (no recent DUI/DWI).
• Fluent reading, speaking, and writing in English.
• Strong communication skills.
• Working cell phone with internet access.

Responsabilités

• Assist clients with personal care and behavior support.
• Follow clients' care plans and goals.
• Accompany clients in recreation and activities.
• Implement plans as written and track data daily.
• Promote client independence and community engagement.
• Attend and participate in team meetings as needed.
