Qualifications

• Must pass Department of Human Services background check.
• Must have a valid driver's license and vehicle insurance.
• Able to meet the physical demands of the job and work varied shifts.

Responsibilities

• Assist clients with daily living activities, personal cares and routine tasks.
• Plan and participate in recreation, leisure and community activities.
• Drive to appointments and accompany clients using a company vehicle.
• Maintain client records and document program data in a timely manner.
• Support client in building community relationships and implementing CSSP/CSSP-A.
