Responsibilities

• Implement and monitor plans of care for persons supported, including assessments and life plans.
• Provide direct care and assistance with daily living activities, including personal hygiene, meals, medication administration, and transportation.
• Facilitate community outings and social activities to promote inclusion and independence.
• Use person-centered approaches and positive behavior support strategies to address challenging behaviors.
• Maintain accurate records, progress notes, and incident reports; ensure regulatory compliance.
• Collaborate with interdisciplinary teams to develop and implement comprehensive care plans.
