Qualifications

• High School Diploma or equivalent.
• Minimum age 18.
• Meet all pre-employment screening requirements.
• Valid driver’s license.

Responsibilities

• Assist individuals in identifying and communicating requests for services and support.
• Promote healthy lifestyle and wellness practices; prevent illness and accidents.
• Foster self-advocacy and support independent living skills.
• Provide transportation to appointments and community activities; accompany individuals during social outings.
• Document services for accurate records and communication among teams.
• Understand and address mental health and behavioral health challenges; connect to resources.
