Responsibilities

• Coordinates services for client.
• Works with Service Manager to ensure client health needs are met, including nutrition, exercise, and mental/physical health.
• Coordinates with Service Manager regarding client needs/concerns.
• Monitors medications; documents intake, counts, and adherence per supervisor.
• Follows financial procedures for client accounts and supervised shopping.
• Provides transportation for daily activities as needed.
• Completes Medicaid transportation billing paperwork as allowed.
• Maintains professional conduct and confidentiality.
• Assists with personal hygiene, dressing, laundry, and meal preparation as required.
• Completes documentation such as progress notes and reports.
• Maintains facility and grounds in good condition.
• Attends required training sessions; may travel.
