Qualifications

• High school diploma or GED (or equivalent) required; 1–3 months of related experience or training acceptable.
• Ability to read and comprehend simple instructions, short correspondence, and memos; ability to write simple correspondence.
• Basic math and computer skills to compute rates, percentages, and interpret simple graphs.

Responsibilities

• Coordinate services for clients.
• Ensure quality health care for clients with Service Manager; monitor nutrition and health.
• Monitor medications, document usage, and assist with billing as needed.
• Provide transportation for daily activities; complete Medicaid transportation paperwork.
• Assist with personal hygiene, dressing, meals, and daily living tasks.
• Maintain documentation forms (progress notes, reports) and facility upkeep.
• Maintain van upkeep and conduct monthly emergency drills.
• Attend required trainings and follow HIPAA and confidentiality policies.
