Qualifications

• Direct support to individuals in residential or program settings following person-centered plans.
• Accurate documentation of progress and adherence to confidentiality.
• Engage in socialization, health maintenance, medications administration as assigned.

Responsibilities

• Deliver support according to individual service plans, including socialization and development.
• Assist with daily activities: personal care, housekeeping, transportation, community integration, shopping.
• Maintain records and logs, review them for changes in service plans.
• Communicate with nurses, supervisors, and family members as appropriate.
