Job summary

Continuum of Care in Norwich, CT is seeking a Direct Support Professional to assist individuals with intellectual disabilities in-home and in the community. You will provide ADL support, personal care, and community participation, maintaining safety and dignity at all times.

You will input data into Therap and coordinate with clinicians, attending meetings as required. Evening, weekend, and overnight shifts may be necessary; valid CT driver’s license is required.

Qualifications

• Bachelor’s or Associate’s degree preferred; high school diploma with relevant experience considered.
• Valid Connecticut driver’s license with clean driving record required.
• Certification as a Direct Support Professional preferred.
• Completion of CPR & First Aid, PMT, and other required certifications through DDS/CoC.
• Computer skills to navigate electronic records, email, and documentation.

Responsibilities

• Establishes rapport and positive working relationships with individuals while maintaining professional standards and boundaries.
• Demonstrates sensitivity and respect for individuals’ dignity.
• Provides face-to-face hands-on support enabling individuals to maintain living in their home.
• Assists with activities of daily living and personal care as needed.
• Supports individuals to participate in routine medical and community activities.
• Input and track daily data in Therap or designated systems.
• Attend and participate in staff and other designated meetings.
• Complete other client care tasks as assigned.
