Job summary

A New Well is seeking a Direct Support Professional (DSP) to provide person-centered support to individuals with physical and/or intellectual disabilities in line with DBHDD standards and DCH Medicaid rules. The DSP promotes independence, safety, dignity, and community inclusion, assisting with daily living skills, self-care, socialization, and meaningful activities in the least restrictive environment.

The role requires strong communication, confidentiality, and the ability to work

Qualifications

• High school diploma or GED required.
• Ability to perform CPR/First Aid.
• Ability to maintain confidentiality and HIPAA.
• English reading, writing and comprehension required.
• Clear and organized documentation of activities.
• Ability to prioritize and multi-task assignments.
• Ability to learn behavioral support strategies.
• Ability to facilitate groups and individual instruction.
• Ability to relate respectfully to individuals with disabilities.
• Promote self-determination and person-centered values.
• Work independently with minimal supervision.
• Strong interpersonal and listening skills.
• Knowledge of professional behavior.
• Maintain professional boundaries and supportive approach.

Responsibilities

• Provide personal care, protective oversight, and supervision.
• Ensure individuals’ safety, health, welfare, and dignity at all times.
• Assist with ADLs (bathing, grooming, toileting, hygiene, dressing, feeding, mobility).
• Assist with medication self-administration per guidelines.
• Support individuals during community outings, appointments, and activities.
• Complete daily, weekly, and monthly documentation accurately.
• Maintain confidentiality and secure handling of protected information.
• Collaborate with families, case managers, clinicians, and other providers.
