Job summary

Responsibilities

• Implement and monitor plans of care including assessments, life plans, behavior support plans, and related documents.
• Provide direct care and assistance with daily living activities, medication administration, and transportation.
• Facilitate community outings, social events, and opportunities to promote independence and meaningful interactions.
• Use person-centered approaches and positive behavior support strategies to address challenging behaviors.
• Maintain accurate daily progress notes and incident reports, ensuring compliance with policies.
• Collaborate with healthcare professionals, therapists, and families to develop comprehensive care plans.
