Job summary

The New York Foundling is seeking a Direct Support Professional to empower individuals with developmental disabilities through personalized care and structured activities. You will support daily living tasks, accompany to appointments, and help promote independence within a compassionate team.

Responsibilities include implementing care plans, documenting progress, and ensuring safety. Strong communication and empathy are essential; CPR/First Aid is preferred. High School or GED required.

Qualifications

• Strong interpersonal and communication skills; patience, empathy, and a genuine desire to support persons with developmental disabilities.
• CPR and First Aid certification preferred; additional training in developmental disabilities, behavior management, or healthcare a plus.

Responsibilities

• Empower persons with developmental disabilities to live fulfilling lives by providing personalized support and fostering independence.
• Implement and monitor plans of care, including assessments, life plans, and behavior support plans.
• Provide direct care and assistance with daily living activities, including personal hygiene, meals, medication administration, and transportation.
• Facilitate community outings and social events to promote inclusion and independence.
• Maintain accurate records, progress notes, and incident reports; ensure regulatory compliance.
• Collaborate with interdisciplinary teams to develop comprehensive care plans.
• Advocate for rights and choices of individuals, acting as custodian and mandated reporter as required.
