Job summary

The New York Foundling is seeking a Direct Support Professional to empower individuals with developmental disabilities to live fulfilling lives by providing personalized support and fostering independence. You will implement care plans, assist with daily living activities such as hygiene, meals, and transportation, and accompany clients to appointments to support health and well-being.

Qualifications

• High school diploma or equivalent required.
• Experience in direct care or human services preferred.
• Valid driver’s license with 6 months of experience and 6 or less points.

Responsibilities

• Implement and monitor care plans including assessments and behavior support plans.
• Provide direct care: daily living activities, hygiene, meals, meds, transportation.
• Facilitate community outings and social events to promote inclusion.
• Use person-centered approaches to address challenging behaviors.
• Maintain records, progress notes, and incident reports per policy.
• Collaborate with healthcare professionals, therapists, and families.
• Advocate for rights and choices; be custodian and mandated reporter.
• Be flexible and responsive to changing needs.
