Job summary

Sevita is seeking Direct Support Professionals to provide compassionate care in residential settings. You will assist individuals with daily living activities, socialization, and health-related tasks, following individualized plans and safety protocols.

Responsibilities include documenting progress, coordinating with families and case managers, and supporting community integration. Candidates should have a high school diploma or equivalent and be at least 18 years old; some prior human services

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers.

Responsibilities

• Directly supports individuals per service plans, including socialization and daily living.
• Documents progress and reviews records, maintaining confidentiality.
• Manages purchases within individual budgets and preferences.
• Maintains professional relationships with individuals, families, guardians, and case managers.
• Reports abuse, neglect, and incidents following Sevita policies.
• Respects rights and uses universal precautions.
• Assists with health care tasks, appointments, and medication administration as needed.
• Supports meal preparation and daily activities, including transportation and community integration.
