Job summary

InCommunity is seeking a Direct Support Professional to support individuals with intellectual and developmental disabilities in home and community settings. You will assist with ADLs, meal preparation, and daily routines while promoting health and safety under ISP guidelines.

The role involves accompanying individuals to appointments, documenting changes, and maintaining a respectful, professional environment across shifts, including evenings and weekends as needed.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in developmental disabilities, mental health, or aging services preferred.
• Valid driver’s license and driving standards may apply.

Responsibilities

• Support individuals in engagement and inclusion in home and community activities.
• Assist with activities of daily living such as cooking, cleaning, grooming and dressing.
• Promote health, monitor daily activities, and follow ISP and medical protocols.
• Accompany to medical appointments and ensure effective communication with providers.
• Maintain safety and cleanliness, assist with meals, and manage transportation as needed.
• Document observations, health concerns, and behavioral changes for supervisors.
