Qualifications

• High school diploma or GED required.
• Excellent communication and listening skills.
• Ability to document clearly and accurately.
• Reliable transportation and valid drivers license required.

Responsibilities

• Provide direct support to individuals with disabilities per ISP and support plans.
• Promote health, safety, and well-being; respond to emergencies and crises.
• Assist with community participation, skills development, and independent living activities.
• Transport individuals to activities and appointments in a personal vehicle.
• Complete documentation, progress notes, and required reports timely.
