Stonecreek Health and Rehab is seeking a Dietary Manager to lead our dietary team and uphold high nutritional standards for residents. Under the supervision of the Dietitian, you will oversee data collection, care plans, meal orders, and staff training to ensure safe, nutritious meals and excellent service.

You will collaborate with the IDT, conduct resident education, and help maintain compliance with food safety regulations. A CDM and healthcare experience are required or preferred.

Qualificações

• Graduate of an accredited Certified Dietary Manager Program.
• Successful completion of the Certified Dietary Manager (CDM) Exam.
• Minimum of one year of experience in healthcare food service.
• Two years of experience working with elderly populations preferred.
• ServSafe Certification preferred.
• Strong communication and organizational skills.
• Ability to lead and manage dietary staff effectively.
• Knowledge of food safety and sanitation regulations.

Responsabilidades

• Assist with initial data collection and screening for participant nutrition assessments.
• Assist with implementation of individualized plan of care for participants as assigned by the RDN.
• Document and report results of observations to Dietitian and Interdisciplinary Team (IDT).
• Update center databases with new/ongoing diet orders and communicate with staff to ensure accuracy and timeliness.
• Meet with participants and families to address nutrition concerns and provide education.
• Communicate concerns to the Dietitian and IDT as needed.
• Review meals, textures, modified diets, hydration, and diet orders with staff.
• Attend Interdisciplinary Team Meetings and participate as needed.
• Monitor plan of care with participants and caregivers and verify implementation.
• Assist in developing dietary policies and procedures and ensure meals are served at proper temperatures.

Essential Functions:

• Under the supervision of the Dietitian, the Certified Dietary Manager will assist with initial data collection and/or screening for participant nutrition assessments.
• Assist with the implementation of individualized plan of care for participants as assigned by the RDN.
• Complete, document, and report to Dietitian and/or Interdisciplinary Team (IDT) results of observations of participants' specific assigned monitoring activities such as: meal intake, weight, food records, etc.
• Process new participant diet orders and all ongoing diet order changes by updating all required center databases and communicate this with all relevant staff to ensure accurate and timely dietary compliance is maintained for each participant.
• Meet with participants and families to address food and nutritional concerns, provide education as needed.
• Proactively and immediately communicates to the Dietitian any shared concerns and/or issues from participants and center staff.
• Reviews center meal service, textured modified diets, thickened liquids, supplements, meal intake documentation, food allergies, and hydration with new employees and on an as-needed basis.
• Reviews new participant dietary needs with dayroom staff monthly and as needed.
• Attends and participates in Interdisciplinary Team Meetings as needed.
• Maintains communication with participant and caregivers in respect to the established nutrition intervention plan of care goals and verifies implementation.
• Monitors and reports participants' status changes as observed that may influence the plan of care (i.e., participant's weight, intake sheets, etc.).
• Assists in developing dietary policies and procedures, monitors quality of food, ensures food is served at appropriate temperatures, and administers participant satisfaction surveys.
• Assists with preparation of meals as needed.

Qualifications:

• Minimum of one (1) year of experience in a healthcare food service setting
• Two (2) years of experience working with elderly populations preferred
• ServSafe Certification preferred
