Job summary

Fresenius Medical Care seeks a Social Worker to provide psychosocial services to in-center and home dialysis patients as applicable. You will work with the interdisciplinary team to promote adjustment and quality of life, educate staff on psychosocial impacts, and participate in QAI activities.

Education includes a Master’s in Social Work with a state license. Typical responsibilities include assessment, care planning, counseling, and referrals to community resources to optimize patient outcomes

Qualifications

• Masters in Social Work from an accredited program.
• Licensed or eligible to practice social work in the state.
• 2–5 years of related experience.

Responsibilities

• Assess patients' psychosocial status and plan care with the interdisciplinary team.
• Provide counseling and supportive services to patients and families.
• Educate patients about treatment options and community resources.
• Assist with advance directives and patient rights discussions as needed.
• Collaborate with staff on quality indicators and patient satisfaction efforts.
• Refer patients to home health, vocational rehab, or other services as appropriate.
