Job summary

Qualifications

• Assesses patients' psychosocial status, strengths and needs impacting rehabilitation.
• Supports care planning with patient and healthcare team to meet goals.
• Provides education and psychosocial support to patients and families.
• Assists with transplant referral information and processes.
• Refers to community resources and vocational rehabilitation.
• Documents MSW interventions per policy.

Responsibilities

• Engages in monthly quality review discussions with the interdisciplinary team.
• Educates patients on treatment options and advance directives.
• Discusses DNR status with physician and nurse per policy.
• Ensures documentation and adherence to FMCNA Social Work Policy.
• Supports patient rights, grievances, and non-discrimination policies.
