Job summary

Qualifications

• MSW degree and state license required.
• Experience delivering psychosocial care in a medical setting is preferred.

Responsibilities

• Assess patients' psychosocial status and participate in care planning with the medical team.
• Provide supportive counseling and education to patients and families.
• Refer patients to community resources and vocational rehabilitation as appropriate.
• Educate staff on psychosocial aspects of Chronic Kidney Disease and dialysis.
• Assist with transplant education and waitlist management as applicable.
