Qualifications

• Experience in dialysis or chronic kidney disease a plus.

Responsibilities

• Assess patients’ psychosocial status, strengths and needs as part of comprehensive patient assessment.
• Participate in care planning with patient and interdisciplinary team to meet rehabilitation and treatment goals.
• Provide counseling, education and referral support related to transplant waitlist and community resources.
• Coordinate with Insurance Coordinators to address financial barriers and assist with access to resources.
• Support quality improvement efforts and participate in team meetings to monitor outcomes.
