Job summary

Dialysis Clinic, Inc. in Alabama is seeking a Dialysis Social Worker to support the psychosocial needs of our dialysis patients.

This full-time role covers clinics in Cullman and Lawrence counties with a Monday–Friday 8am start and weekends off. You will help patients access community resources, advocate for healthcare benefits, educate on ESRD options, coordinate translation services, and work with an interdisciplinary team to improve quality of life.

Qualifications

• MSW degree required
• Licensed in the state of practice
• New MSW graduates welcome, job shadow opportunities available
• Renal care experience preferred, not required

Responsibilities

• Provide patient support to address emotional, financial and lifestyle changes due to chronic kidney disease
• Assist patients with community resources for nutrition, emotional support, transportation and housing
• Advocate for access to healthcare and other benefits for patients
• Educate patients and families about ESRD treatment options including dialysis, transplant and medical management
• Help patients compare treatment options to make informed decisions
• Coordinate translation services for patients and caregivers
• Assess family and support systems and refer for additional services as needed
• Provide counseling to patients and families to cope with kidney failure and rehabilitation goals
• Develop individualized care plans with the care team based on psychosocial assessment
• Collaborate with interdisciplinary team to adjust care plans as needs evolve
• Educate team about emotional aspects of dialysis
• Advocate for patients at clinic, community and state/federal levels
• Serve as ESRD Network liaison
• Engage in data-driven quality improvement to improve outcomes
