Dialysis Clinic, Inc. is seeking a Social Worker in Grants, NM to address the psychosocial needs of dialysis patients. You will assess, plan, and connect patients with community resources while supporting families managing chronic kidney disease.

The role offers full-time hours, weekday schedules, and a comprehensive benefits package including medical, dental, vision, retirement, and PTO. Prior renal care experience is a plus, but new MSW graduates are welcome to apply.

Qualifikationen

• MSW required and NM licensure
• New MSW graduates welcome
• Renal care experience preferred if possible

Aufgaben

• Provide psychosocial support to patients addressing emotional, financial and lifestyle changes from CKD
• Assist patients with accessing community resources for nutrition, transportation, and housing
• Advocate for patients to access healthcare and benefits
• Educate patients and families on ESRD treatment options including dialysis, transplant and medical management
• Help patients compare treatment options to make informed decisions
• Coordinate translation services for patients and caregivers
• Assess family support and refer for additional services as needed
• Provide counseling to patients and families to cope with kidney failure and dialysis goals
• Develop individualized care plans with the interdisciplinary team to improve quality of life
• Collaborate with team to adjust care plans as patient needs evolve
