Qualifications

• MSW degree and state licensure required.
• Renal care experience preferred but not required.
• Strong communication and listening skills essential.
• Collaborative team player with care coordination abilities.

Responsibilities

• Provide patient support addressing emotional, financial and lifestyle changes.
• Assist patients in accessing community resources and benefits.
• Educate patients and families about ESRD treatment options.
• Coordinate translation services for patients and caregivers.
• Develop and implement individualized care plans with the care team.
