Qualifikationen

Aufgaben

• Provide patient support addressing emotional, financial and lifestyle changes from CKD.
• Assist patients with community resources for nutrition, transportation and housing.
• Advocate and assist patients in accessing healthcare and benefits.
• Educate patients/families on ESRD treatment options, including dialysis/transplant.
• Coordinate translation services for patients and caregivers.
• Develop individualized care plans with interdisciplinary team.
