Qualifications

• LMSW or LCSW licensed in New York state.
• Master’s degree in social work from CSWE-accredited program.
• One year of dialysis experience preferred; training possible.

Responsibilities

• Conduct psychosocial assessments for ESRD patients and families.
• Document patient status quarterly and as needed.
• Participate in patient care plan meetings and coordinate referrals to community resources.
• Develop and present psychosocial education materials.
• Maintain confidentiality and privacy of all patients.
