Will provide psychosocial assessments, counseling, and education, while developing care plans and coordinating transfers. You will also handle ESRD Network and CrownWeb reporting as part of a collaborative, professional environment.

Qualifications

• LMSW or LCSW license required.
• Experience in psychosocial assessment and counseling.
• Healthcare social work experience preferred.

Responsibilities
