Job summary

Brooklyn Gardens Dialysis is seeking a social worker to provide psychosocial support to ESRD and CKD patients and their families. The role includes assessments, benefits counseling, and care coordination within a multidisciplinary team.

Candidates must have dialysis experience, an MSW with clinical focus, and NYSED licensure. This position emphasizes patient-centered care and linkage to community resources in a busy dialysis setting.

Qualifications

• Must have experience working with dialysis patients.
• Master’s Degree in Social Work with a specialization in clinical practice and NYSED certification.
• Must be Licensed to practice social work.
• Qualified by credentials and experience to meet Federal ESRD program requirements.

Responsibilities

• Conduct psycho-social assessments for each patient and develop social work treatment plans.
• Advise on benefits under Medicare/Medicaid/other insurance options and assist patients in accessing them.
• Assist patients and families in interpreting benefits and obtaining entitlements.
• Participate in multi-disciplinary conferences and care planning.
• Utilize crisis intervention techniques as needed.
• Refer to public/private community agencies and support patient transportation and services.
• Liaise with Dialysis Center, hospitals, and nursing homes; coordinate patient transfers.
