Responsibilities include psychosocial assessment, patient support and counseling, patient education, care planning, facility transfers, and ESRD Network/Crownweb reporting. LMSW/LCSW is required; clinical dialysis experience is preferred, and training is available.

Qualifications

• Current LMSW or LCSW licensure.
• Part-time availability.
• Experience in dialysis or ESRD preferred.
• Willing to train for dialysis environment.

Responsibilities

• Psychosocial assessment of patients.
• Patient support and counseling.
• Patient education and counseling.
• Develop and monitor care plans.
• Coordinate facility transfers as needed.
• Submit ESRD Network/Crownweb reporting.
