Job summary

Renalliance Group in New York is seeking a highly motivated part-time Renal Social Worker for a state-of-the-art dialysis center. Applicants must hold a current LMSW/LCSW and be prepared to join an interdisciplinary team providing psychosocial support to patients.

Responsibilities include psychosocial assessment, patient counseling, education, care planning, and coordinating facility transfers. Knowledge of ESRD Network/Crownweb reporting is desirable.

Qualifications

• Must have LMSW or LCSW licensure in the state.
• Clinical social work experience with ESRD or dialysis patients preferred.
• Comfort with psychosocial assessments and patient counseling.

Responsibilities

• Perform psychosocial assessments and psychosocial support.
• Provide counseling to patients and families.
• Educate patients on dialysis care and treatment options.
• Develop and implement care plans with the interdisciplinary team.
• Coordinate facility transfers as needed.
• Report ESRD Network/Crownweb data as required.
