Qualifications

• Must hold LMSW or LCSW licensure in the state.

Responsibilities

• Provide patient support and counseling.
• Educate patients and families about care plans.
• Develop care plans and coordinate facility transfers.
• Document reporting to ESRD Network/CROWNWeb as required.
