Job summary

Fresenius Medical Care is seeking an entry level MSW to provide psychosocial services to patients treated by the facility, including in-center and home dialysis where applicable. You will collaborate with the interdisciplinary team to promote adjustment and quality of life for patients and families, and to inform and educate staff on psychosocial impact of Chronic Kidney Disease.

Qualifications

• Master's degree in Social Work required.
• Must have state required license and licensure.
• Meets applicable scope of practice board and licensure requirements in effect in the State employed.

Responsibilities

• Assess patients' psychosocial status, strengths and needs as part of comprehensive assessment.
• Participate in care planning with patient and healthcare team to improve rehabilitation, goals and quality of life.
• Provide monitoring and interventions to help patients adjust to dialysis and achieve psychosocial stability.
• Offer supportive counseling and education regarding transplant referrals.
• Refer patients to community resources and vocational rehabilitation as needed.
• Collaborate with the team on training staff and contributing to Quality Assessment and Improvement activities.
