Job summary

U.S. Renal Care seeks a Licensed Master Social Worker to provide social services to patients and families, promoting psychosocial adjustment to dialysis and rehabilitation. You will work within the interdisciplinary team to support patient care and advocate for resources.

Responsibilities include coordinating care, identifying social needs, educating patients and families, and participating in QAPI and readiness activities to improve outcomes.

Qualifications

  • Master's Degree in Social Work (CSWE accredited) required.
  • Current licensure in applicable state required unless state exemptions apply.
  • Experience in providing social services to dialysis patients preferred.
  • Excellent written and verbal English communication.
  • Basic computer skills; proficiency in USRC clinical applications within 90 days of hire.

Responsibilities

  • Provide social services to patients and families to promote social functioning and adjustment to dialysis.
  • Assist with clinical and operational processes to improve patient health and reduce hospitalizations.
  • Coordinate transient arrangements for patients.
  • Identify psychosocial issues and counsel patients and families.
  • Educate patients and families about resources and program goals.
  • Participate as a member of the interdisciplinary care team and contribute to care planning.
