Job summary

U.S. Renal Care is seeking a Licensed Master Social Worker to provide psychosocial services for patients and families undergoing dialysis. The role emphasizes coordination with the interdisciplinary team, patient education, and advocacy within state regulations.

The candidate should have an MSW (CSWE-accredited) with LCSW preferred, active licensure, and strong English communication skills; computer proficiency is required within 90 days of hire.

Qualifications

• Must have Master’s Degree in Social Work accredited by the CSWE and LCSW preferred.
• Current licensure in applicable state is required unless the state is AZ/PA/Guam.
• Previous experience in providing social services to dialysis patients preferred.
• Demonstrated working knowledge of English; written and verbal communication.
• Basic computer skills including Microsoft Office; proficiency with USRC clinical applications within 90 days.

Responsibilities

• Provide social services to patients and families to promote social functioning and adjustment to dialysis and rehabilitation.
• Assist with clinical and operational processes to improve patient health and minimize hospitalizations.
• Coordinate with interdisciplinary team and identify social agencies and resources for patients.
