Job summary

LiveWell Homecare Agency, LLC seeks a Dialysis Social Worker to join an interdisciplinary team delivering compassionate, patient-centered care to dialysis patients.

You will perform psychosocial assessments, provide counseling, and collaborate with clinicians to support emotional and social needs of patients and families across ages. A master’s in social work and state licensure (LGSW or LICSW) are required or preferred. The role spans neonatal to geriatric populations.

Qualifications

• Master’s degree in Social Work from an accredited program.
• Licensed in good standing as a Licensed Graduate Social Worker (LGSW) or Licensed Independent Clinical Social Worker (LICSW) in the practicing state.
• Healthcare or mental health hospital experience preferred.

Responsibilities

• Perform psychosocial assessments and respond to abuse, neglect, or violence per ethical standards.
• Collaborate with healthcare professionals to develop treatment plans and support emotional, mental, and social needs of patients and their families.
• Provide counseling addressing social, emotional, and economic concerns, across neonatal to geriatric populations.
• Utilize age-appropriate care protocols and understand the impact of illness on patients and families.
• Possible diagnosis (if licensed) and brief therapy to manage psychosocial and economic challenges.
