Job summary

Fresenius Medical Care seeks a Transitional Care Unit Registered Nurse to support patients entering dialysis, coordinating care and education for in-center or home modalities. The RN CAP 2 track allows entry or progression from RN CAP 1, with responsibilities spanning assessment, plan of care, education, and collaboration with the inter-disciplinary team.

The role includes leadership, precepting, and participation in quality improvement initiatives within the facility, under supervision.

Qualifications

• RN with nephrology nursing experience preferred, entry level can qualify via CAP program progression.
• Graduate of an accredited School of Nursing and active state licensure required.
• Current BLS/CPR certification; maintain licensure per state requirements.
• Must meet practice requirements in the state of employment.
• Experience as RN CAP 2 or progression from RN CAP 1 is a pathway described.

Responsibilities

• Provide clinically competent care including assessment, planning, intervention, and evaluation for assigned patients.
• Coordinate care and education for patients and families about dialysis modalities.
• Administer medications and document treatment effectiveness according to protocols.
• Lead and mentor other nursing staff and participate in quality improvement activities.
