Qualifications

• BLS certification required.

Responsibilities

• Provide chronic disease education tailored to a specific patient population.
• Coordinate care with PCPs, specialists, and hospitals to optimize outcomes.
• Educate patients and families considering cultural, language, and readiness factors.
• Document care services in charts per policy and regulations.
• Identify opportunities to improve quality and reduce hospital admissions.
