Job summary
ChenMed is seeking a Patient Educator, Specialty (RN) to collaborate with medical teams and deliver disease education to a specialized patient group. The role focuses on improving outcomes, engaging patients in care decisions, and delivering clinical education tailored to cultural and linguistic needs.
On-site daily across Tidewater offices (Virginia Beach, Norfolk, Portsmouth, Newport News, Hampton). Requires strong nursing knowledge, teaching experience and ability to travel up to 40%
Qualifications
- Active RN license required; Nursing Compact license preferred.
- Minimum of 3 years' dialysis experience.
- Teaching/training experience required (clinical or patient education preferred).
- Experience with geriatric patients highly desirable.
Responsibilities
- Provides chronic disease education relative to quality care for a specific patient population.
- Monitors chronic disease program performance across multiple medical centers.
- Assists with the development of strategies for quality improvement and management related to disease education.
- Educates patients and/or family members by understanding cultural and religious practices, barriers, and readiness to learn.
- Coordinates care with PCPs, specialists and vendor services to reduce hospitalizations and ED visits.
