Job summary

ChenMed LLC is seeking an RN-based Patient Educator, Specialty, to deliver chronic disease education, coordinate care, and monitor program outcomes across multiple centers. The role emphasizes culturally competent education for patients and families, documentation, and ongoing quality improvements.

Ideal candidates will hold an active RN license with dialysis experience, BLS certification, and a background in teaching or patient education. Travel up to 40% nationwide may be required.

Qualifications

• RN license required; Dialysis experience preferred.
• Teaching/training experience in clinical or patient education is required.
• BLS certification required; CNE certification a plus.

Responsibilities

• Provides chronic disease education relative to quality care for a specific patient population.
• Coordinates care between PCPs, community specialists, and vendor services including hospitals.
• Monitors chronic disease program performance across multiple medical centers.
• Identifies gaps and implements improvements in patient and quality outcomes.
• Educates patients and/or family members considering culture, language, and readiness to learn.
• Documents patient care services in charts and department records.
