Job summary

Fresenius Medical Care is seeking an RN CAP 2 for our outpatient dialysis program. The role focuses on assessing, planning, and delivering high-quality hemodialysis care to an assigned patient group, while coordinating with the interdisciplinary team to support treatment goals and improve outcomes.

As part of the Clinical Advancement Program, the RN CAP 2 may mentor new staff, participate in quality improvement, and ensure compliance with nursing practice standards, policies, and state

Qualifications

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure.
• Current CPR BLS Certification.

Responsibilities

• Provide and coordinate clinically competent care including assessment, planning, intervention, and evaluation for assigned patients.
• Collaborate with RNs, LVN/LPNs, and technicians; delegate tasks as appropriate.
• Perform ongoing data collection, analyze patient data, adjust treatment plans, and notify the care team as needed.
• Educate patients and families on care plans, treatments, and transplant options.
• Direct daily delivery of safe hemodialysis therapy and monitor patient responses.
