Job summary

Ennoble Care in Union County, New Jersey, seeks a full-time Care Coordinator to join partnered dialysis clinics. You will engage patients on-site, coordinate care with providers and facility staff, and support care transitions while maintaining thorough documentation.

Candidates should have clinical certification (PCT, CNA, MA/CMA, LPN, RN or comparable) and strong organization, communication, and computer skills; dialysis experience preferred.

Qualifications

• Legally authorized to work in the United States
• Active, unrestricted clinical certification or licensure (PCT, CNA, MA/CMA, LPN, RN or comparable)
• Dialysis-specific certification preferred
• Experience coordinating provider schedules within a clinical setting preferred
• Minimum 1 year dialysis/nephrology patient experience preferred
• Strong communication and organizational skills

Responsibilities

• Provide on-site engagement with dialysis patients to support care plans and transitions
• Coordinate care using Ennoble Care’s EMR to ensure continuity and timely follow-up
• Schedule patient visits and support daily clinic workflow
• Collaborate with providers and facility staff for integrated patient-centered care
• Maintain accurate, timely documentation and communicate with the care team
