Job summary

Advocate Health is seeking a dedicated Diabetes Education Specialist to provide DSMES per ADA/ADCES guidelines, delivering education to patients and families in inpatient and outpatient settings.

You will assess learning needs, lead individual and group sessions, and coordinate care plans with the healthcare team while staying current with diabetes management advances and payer requirements.

Qualifications

• Current license or certification to practice as an RN or RD in the applicable state.
• CDCES preferred; if not certified, certification within two to three years preferred.
• Degree in Nursing or Dietetics meeting CDCES eligibility; BSN preferred; master's in Dietetics if licensure obtained after 01/01/2024.

Responsibilities

• Provide DSMES per ADA/ADCES standards.
• Assess learning needs, goals, and barriers; tailor education.
• Lead individual and group diabetes education sessions (inpatient or outpatient).
• Provide medical nutrition therapy when RD; ongoing follow-up for diabetes management.
• Collaborate with healthcare team to implement comprehensive care plans.
• Stay current on diabetes care developments and CMS/payor guidelines.
• Document services per CMS/payor requirements.
