Job summary

Capital Health is seeking a capable Diabetes Education RN to provide clinical management and self-management education to diabetes patients. You will collaborate with physicians and staff to design, deliver and adapt education programs for diverse patient populations in an outpatient setting.

The role requires an accredited nursing degree, RN licensure in NJ, and the goal of obtaining CDE within one year, along with BLS.

Qualifications

• Graduate of an accredited nursing program.
• Must attain Certified Diabetes Educator (CDE) certification within one year of hire.
• AHA BLS – Healthcare Provider certification; NJ RN licensure required.

Responsibilities

• Provide clinical management and diabetes self-management education to patients.
• Develop and update educational materials and programs.
• Create individualized care plans based on assessments.
