Job summary

Capital Health in Trenton, NJ is seeking a Diabetes Educator RN for a full-time day shift in the Maternal Fetal Medicine program. You will provide diabetes self-management education, plan and present patient- and provider-focused education, and help develop materials that meet diverse learning needs.

The role requires RN licensure in NJ, BLS certification, and the goal of obtaining CDE within 12 months. Strong communication and collaboration across teams are essential.

Qualifications

• Graduate of an accredited nursing program.
• Must attain Certified Diabetes Educator (CDE) certification within one year of hire.
• AHA BLS - Healthcare Provider, RN license in NJ.
• Strong knowledge of endocrine/diabetic care.

Responsibilities

• Provides clinical management and diabetes self-management education to diabetes patients in conjunction with LIPs.
• Plans and presents educational opportunities to meet learning needs of patients and providers.
• Maintains educational materials and ensures compliance with ADA and regulatory standards.
• Incorporates nutritional assessments and personalized meal planning in coordination with a dietitian.
