Job summary

THOM Thomas Hospitals in Charleston, WV is seeking a Certified Diabetes Educator to develop, manage, and coordinate all aspects of the outpatient diabetes program. You will lead outreach to physician practices, perform patient assessments, deliver education, and drive quality improvement while maintaining ADA certification and coordinating the Diabetic Institute.

The role requires CDCES certification and at least one year of health/fitness experience; proficiency with Excel and Word is preferred.

Responsibilities

• Identifies education needs of the community and people with diabetes.
• Delivers education and evaluates patient outcomes.
• Maintains American Diabetes Association certification where applicable.
• Participates in community outreach and educational support.
• Communicates with primary care providers and the multidisciplinary team.
• Leads outpatient diabetes program planning, implementation and evaluation.
• Coordinates the Diabetic Institute operations.
• Compiles materials, summarizes data, and drafts manuals.
• Supports performance improvement activities to ensure quality care.
