Job summary

Wheeling Hospital, Inc. seeks a Diabetes Education Specialist to develop, manage, and coordinate all aspects of the outpatient diabetes program, including outreach to physician practices as referral sources for the diabetes program.

The candidate will provide education, coordinate care, and work with a multi-disciplinary team to improve patient outcomes, requiring CDCES certification and at least 1 year of health/fitness experience.

Qualifications

Responsibilities

CORE DUTIES AND RESPONSIBILITIES

• Identifies education needs of the community and those with diabetes.
• Attends diabetes advisory committee meetings as assigned.
• Performs patient assessment, delivers education and evaluates patient's obtainment of goals.
• Assists in maintaining the American Diabetes Association certification as applicable.
• Participates in community outreach and educational support.
• Advocates on behalf of the patient and families.
• Communicates with primary care providers, members of the multi-disciplinary team relevant questions concerns, and patient outcomes.
• Participates in performance improvement activities to assure quality care.
• Provides administrative support and coordinates the management and operations of the Diabetic Institute
• Leads the planning, implementation and evaluation of the Outpatient Diabetes and Nutrition Education Program.
• Compiles reference materials, summarizes data and prepares reports, and drafts manuals and materials.
